The skills problem

Laparoscopy removes direct tactile feedback, reduces depth perception to a two-dimensional image, and reverses instrument movement around the port fulcrum. These are perceptual-motor adaptations that take deliberate practice to acquire.

Simulation exists to separate that adaptation from the cognitive demands of the operation itself, so trainees are not learning instrument handling and procedural decision-making simultaneously in a live case.

Modalities

Box trainers place real instruments and a camera over physical tasks — peg transfer, pattern cutting, ligating loops, suturing. They are inexpensive, high-repetition and remain the backbone of most curricula.

Virtual reality platforms add automated metrics such as path length, economy of movement and error counts, which allow objective progression criteria. Hybrid and augmented models combine physical tissue handling with digital assessment.

What makes a curriculum work

Evidence favours proficiency-based progression: trainees continue until they meet a defined performance benchmark, rather than completing a fixed number of hours. Distributed practice across sessions outperforms massed practice, and structured feedback is essential — unguided repetition can entrench poor technique.

Device-specific rehearsal

Generic skill does not transfer automatically to a specific energy device. Rehearsing sonotrode or instrument selection, irrigation settings and tissue response at a dry station before first clinical use is a short, high-value addition to any curriculum.
